Effects of a Pilates program in patients with chronic low back pain experiencing knee pain or discomfort: Quasi-experimental single-group study

Abstract

Background: Low back pain is a common issue affecting 19.9% of the population annually, with a higher incidence in women. It is often associated with knee disorders, impacting daily activities and leading to a decrease in quality of life. Objectives: To assess the effectiveness of the Pilates Method in improving the quality of life for patients with chronic low back pain experiencing knee pain or discomfort, determine the frequency of knee pain or discomfort, evaluate the impact of treatment on 12 daily life activities, and analyze adherence and dropout rates when treatment is carried out in a home environment. Method: A quasi-experimental study was conducted with 27 patients from the University Hospital of Henares, with an average age of 58.1 years, predominantly women (85.2%). Pilates sessions were conducted twice a week for three months, each lasting 60 minutes. After the three months, autonomous practice at home continued for six months, maintaining the same number and duration per week as the directed sessions. Results: Of the 24 patients who completed the program, a significant 12.24% decrease in the total score of the 12 daily life activities was observed. Significant changes were noted in 10 of the 12 evaluated activities (p<.05), with improvements ranging from 4.15% to 16.66%. The effect size was large (Cohen's d = 0.938). The mean difference in total score between the start and end of hospital treatment at three months was 5.875 points. A clinically meaningful change was identified (>4.4 points), close to the minimally clinically significant difference (6.1 points). Despite these positive results, a high home dropout rate (58%) was highlighted. Conclusions: The results support the therapeutic effectiveness of the Pilates method, significantly improving the quality of life for patients with chronic low back pain experiencing knee pain or discomfort. However, they also indicate a concerning trend of high treatment dropout rates when conducted autonomously at the patients' homes.
