Eficácia das técnicas instrumentais oscilatórias de desobstrução brônquica no tratamento da Doença Pulmonar Obstrutiva Crônica, uma revisão sistemática

Introdução. Técnicas de desobstrução brônquica utilizando dispositivos instrumentais oscilantes utilizados na abordagem de pacientes com DPOC podem melhorar o padrão de tosse e promover o fluxo expiratório, facilitando a expulsão de secreções, evitando atelectasias, promovendo ventilação mecânica, oxigenação e manutenção de via aérea patente. Métodos: Foi realizada uma revisão sistemática seguindo os critérios de inclusão a partir de uma busca em bases de dados eletrônicas como: PubMed, Science Direct, Scopus, Springer, Google Scholar, entre o período de 01/05/2023 a 01/02/2024 em inglês, Idioma espanhol e português, incluindo estudos como Ensaios Clínicos Randomizados e estudos de coorte. Resultados: Foram incluídos 14 artigos, 100% em inglês. O número de participantes foi de 3.105, com idades entre 35 e 85 anos, 50,2% dos participantes eram mulheres. Oscilação Oral de Alta Frequência (HFCWO®) foi o dispositivo mais utilizado 35,7% n=5, seguido do Flutter® 28,6% n=4. O tempo de intervenção variou entre 1 a 12 semanas, de 3 a 7 vezes por semana, entre 1 a 4 sessões diárias, com duração entre 10 a 30 minutos, entre 10 a 20 Hz Conclusões: O uso do HFCWO pode ter um impacto positivo em. reabilitação pulmonar, especificamente nas variáveis de função pulmonar e oxigenação, quando implementada como terapia complementar à mobilização precoce, exercício terapêutico e técnicas respiratórias para desobstrução brônquica.
