Efecto del entrenamiento muscular inspiratorio en pacientes sometidos a cirugía torácica y abdominal alta: Una revisión integrativa

Resumen

El entrenamiento de los músculos respiratorios puede mitigar la disfunción de los músculos respiratorios resultante de las cirugías torácicas y abdominales superiores. Esta revisión integradora tuvo como objetivo describir los efectos del entrenamiento de los músculos inspiratorios (IMT) sobre la fuerza de los músculos respiratorios en pacientes post-cirugía torácica y abdominal superior. Las búsquedas en las bases de datos PubMed, Lilacs y Scielo utilizaron descriptores científicos. Se incluyeron ensayos clínicos controlados y no controlados publicados en los últimos diez años (de 2013 a 2023) en inglés o portugués, correspondientes al objetivo del estudio. Tres investigadores independientes seleccionaron y analizaron los estudios. Se encontraron veintiocho ensayos. De ellos, se analizaron siete. La mayoría de los estudios involucraron a personas menores de 40 años, preobesas y personas que se sometieron a cirugía cardíaca/torácica (revascularización miocárdica, reemplazo valvular o ambas). El entrenamiento de los músculos inspiratorios estuvo presente en todos los ensayos incluidos. En cuanto a la duración, dos estudios realizaron la intervención con GIM sólo en el postoperatorio inmediato y el primer día del postoperatorio, un estudio la realizó por tiempo indefinido, según el período de hospitalización, tres estudios realizaron GIM entre siete y doce semanas, y uno El estudio amplió el tiempo de formación hasta por 12 meses. Finalmente, el GIM está indicado durante la hospitalización para promover la ganancia de fuerza de los músculos inspiratorios en pacientes después de cirugías torácicas y abdominales altas. También destacamos la necesidad de estandarización de protocolos y estrategias terapéuticas en la rehabilitación respiratoria de estos pacientes. Por tanto, la fisioterapia respiratoria necesita integrar el GIM en la rutina de rehabilitación de estos pacientes, con el objetivo de prevenir y tratar las complicaciones respiratorias y reducir el tiempo de hospitalización y la morbilidad y mortalidad.
